Payment
Acceptable forms of payment are cash or check, and payment is due at the time of service. Credit cards (MC
and Visa) are accepted with an additional service fee.

Covid-19 Safety Policies
An ultraviolet C steralizing light was installed in the office air conditioning system to improve
disinfection of the office. This is what hospitals use to protect against bacteria and viruses. Masks will
be worn by Tenley Fukui, LPC and her clients when social distancing is not possible. Tenley Fukui, LPC will
wear gloves if she does biofeedback with a client. These measures are taken in addition to the regular
cleaning and disinfecting of the office to keep everyone safe.
Insurance
I am not contracted as a provider for any insurance company. Full payment is expected at the time of
service. I can provide a receipt you can file for possible reimbursement by insurance.
Cancellation policy
Please contact me at least 24 hours in advance if you must cancel your scheduled therapy appointment. If you
do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in
advance, you will be required to pay the full cost of the session. Please leave a voicemail message or a
text message if I am not available, stating that you need to cancel or reschedule an appointment.
